Fitness Isn’t an Overnight Sensation

CARL FOSTER, an exercise physiologist at the University of Wisconsin, La Crosse, was amused by ads for a popular piece of exercise equipment. Before-and-after photos showed pudgy men and women turned into athletes with ripped bodies of steel. And it all happened after just 12 weeks of exercising for 30 minutes three times a week. Then there was the popular book, with its own before-and-after photos, promoting a program that would totally change your body in six weeks with three 20-minute exercise sessions a week.

There are many examples of people who took up exercise and markedly changed their appearance. But how long does it take? And how much time and effort are required? Six weeks sounded crazy to Dr. Foster.

“We said: ‘Wait a minute. You can’t change yourself that much,’ ” Dr. Foster said. So he and his colleagues decided to experiment. Suppose they recruited sedentary people for a six-week exercise program. Would objective observers notice any changes in their bodies?

The plan was to photograph volunteers wearing skimpy bathing suits and then randomly assign them to one of three groups: cardiovascular exercise, weight lifting or control. Six weeks later, they would be photographed again.

Their heads would be blocked out of the photos, which would be shuffled. Then the subjects and judges would rate the body in each photo on a scale of 1 to 10, with 10 being spectacular.

The volunteers were men, age 18 to 40 (the university’s human-subjects review board looked askance at having women photographed and rated like that). And they were sedentary. “These were people who were just sort of dumplings,” Dr. Foster said.

Results were not surprising. The subjects rated themselves more highly than anyone else rated them, and female panelists rated the subjects lower than the male subjects or panelists rated them. But, over all, the subjects’ ratings barely changed, if at all, after their exercise program. And neither did objective measures, like weight or percentage of body fat, or waist size or the size of the bicep or thigh.

Exercise physiologists approach the whole new year, new you, total body transformation mania with a jaundiced eye. Yes, they said, people can change the way they look. But not overnight.

“I think it’s pretty clear,” said William Kraemer, a kinesiology professor at the University of Connecticut. Often the promises are just marketing, he said. “A lot of times when you are dealing with health clubs, they are trying to get new members who have made New Year’s resolutions.”

“To make a change in how you look, you are talking about a significant period of training,” Dr. Kraemer said. “In our studies it takes six months to a year.” And, he added, that is with regular strength-training workouts, using the appropriate weights and with a carefully designed individualized program. “That is what the reality is,” he said.

And genetic differences among individuals mean some people respond much better to exercise than others, said Dr. Mark Tarnopolsky, an exercise researcher at McMaster University in Hamilton, Ontario. He added that although he does not think the before-and-after photos in ads are doctored, most people will not change so markedly no matter how hard or long they work. “I believe they are taking the top one or two people out of thousands,” Dr. Tarnopolsky said.

People who did change their bodies say six months is a bare minimum to see real change.

Schuyler Antane, 43, a research scientist, is one. He began in January 2006 with a diet, which meant, he said, “letting go of the foods that taste good, but are wicked evil. And no more beer.”

In three months, he had lost 10 pounds and was down to 190 pounds on his 5-foot-8-inch frame. Then he read a magazine article on 5-kilometer races and decided to try to run. He could run for only five minutes when he started, and it took two months to train for his first race. But he kept at it and improved. Within six months, he weighed about 150 pounds. Then he added bicycling and swimming, becoming a triathlete. That, he said, got him to his fighting weight of 140 to 145 pounds.

“My beer belly is long gone,” he said. “The only flab in my midsection is excess skin, but I am not vain enough to have an operation.”

Now, said Mr. Antane, who runs with a group in Princeton on Thursday nights, “everything changed — my outlook on life, who I hung out with, how I felt about myself.”

Jim Lisowski, 45, the owner and chief executive of SciTec, a research and development company in Montgomery, N.J., said he had let himself slip out of shape, going from 189 pounds to 225 pounds. He is 5-foot-10 1/2. Then his wife bought a joint membership at a gym within walking distance of his office. At first, he went sporadically, but he decided to get serious after about three years.

That was the end of February 2005. By the start of 2006, Mr. Lisowski, who goes to one of my gyms and whose company employs one of my best friends, was a changed man. He weighed 184 pounds and had a muscular, utterly transformed body. He did it with a routine he continues to this day — working out five or six days a week with more than an hour of hard cardio, first on an elliptical cross-trainer and then a rowing machine followed by lifting weights for about an hour.

“My approach was to get fit,” Mr. Lisowski said. “I knew I would lose weight.”

The nine months or so that it took to lose the weight and gain strength and endurance seemed fast to him. He attributes it to the fact that he had been fit before he let himself go, and to his attitude.

“You can go to a gym and spend time there and not make changes,” he said. “You’ve got to break a sweat, you have to increase the weights. You’ve got to challenge yourself.”

Then there’s Charles Reilly, a federal prosecutor in Manhattan and a marathon runner who took a 10-year hiatus from the sport when he joined his local school board. He just did not have time to exercise, he said. Along with exercising less, he ate more. Soon he ballooned from 159 pounds to 282. “It came on gradually, but it came on,” Mr. Reilly said of the weight.

On April 18, 2005, he had his last school board meeting — he’d decided not to run for any more terms. Eight days later, he went out for a run.

“After half a mile, I had to stop and walk,” Mr. Reilly said. But he kept trying. A month later, he could run three miles without stopping. After three or four months, he says, he could run for five miles. By the end of 2006, he ran 10 miles. In the meantime, he also changed his diet. “My goal was to lose 100 pounds,” Mr. Reilly said. He did it, hitting his goal on Feb. 3, 2007, in a little over 21 months.

Mr. Reilly continues to run and has maintained his lower weight. Many who knew him when he was on the school board no longer recognize him, he said. “They do a double take and say, ‘Is that you?’ ”

But, Mr. Reilly said, he never believed those ads saying you can transform yourself almost overnight.

“It’s not really possible,” he said.

Don’t Starve a Cold of Exercise

YOU have what seems to be a really bad cold. You are coughing and sneezing, and it is hard to breathe.

Should you work out?

And if you do, should you push yourself as hard as ever or take it easy? Will exercise have no effect, or make you feel better or worse?

It is a question, surprisingly enough, that stumps many exercise physiologists and infectious disease specialists.

“That question has not been actually studied,” said Dr. Aaron E. Glatt, a spokesman for the Infectious Diseases Society and the president of New Island Hospital in Bethpage, N.Y.

Many avid exercisers make up their own rules, and it seems that many of them, like Dr. Michael Joyner, an exercise researcher at the Mayo Clinic who is a swimmer and runner, decide to keep exercising if they possibly can.

“I can tell you that unless I am really wiped out, I still work out but maybe scale back a bit,” Dr. Joyner said. “I think that would be the answer from most relatively hard-core, old-school types.

“If I have an obvious fever and muscle aches,” he continued, “I do very little or take a day or two off, but I really have to be in a bad way to skip more than that.”

Dr. Bill Schaffner, chairman of the department of preventive medicine at Vanderbilt University and a member of the board of directors of the Infectious Diseases Society, said he was unaware of any studies that addressed the issue.

Dr. Schaffner described himself as a jogger who runs a few miles most days and goes to a gym for resistance training. And, he said, he continues his workouts when he has a cold.

Exercise, he said, makes him feel better. He speculates that perhaps it is because his blood vessels are dilated when he exercises.

“I think exercise pushes me along a route to recovery,” Dr. Schaffner said. “Of course, I recognize that I might have been on a route to recovery anyway. But I can’t think of a reason why exercise would affect you adversely.”

It turns out that, even though they were unaware of them, the strategies of people like Dr. Schaffner and Dr. Joyner are actually supported by two little-known studies that were published a decade ago in the journal Medicine & Science in Sports & Exercise. Results from the studies were so much in favor of exercise that the researchers themselves were surprised.

The studies began, said Leonard Kaminsky, an exercise physiologist at Ball State University, when a trainer at the university, Thomas Weidner, wondered what he should tell athletes when they got colds.

The first question was: Does a cold affect your ability to exercise? To address that, the researchers recruited 24 men and 21 women ages 18 to 29 and of varying levels of fitness who agreed to be deliberately infected with a rhinovirus, which is responsible for about a third of all colds. Another group of 10 young men and women served as controls; they were not infected.

At the start of the study, the investigators tested all of the subjects, assessing their lung functions and exercise capacity. Then a cold virus was dropped into the noses of 45 of the subjects, and all caught head colds. Two days later, when their cold symptoms were at their worst, the subjects exercised by running on treadmills at moderate and intense levels. The researchers reported that having a cold had no effect on either lung function or exercise capacity.

“I was surprised their lung function wasn’t impaired,” Dr. Kaminsky said. “I was surprised their overall exercise performance wasn’t impaired, even though they were reporting feeling fatigued.”

He said he also tested the subjects at different points in the exercise sessions, from moderate to intense effort, and found that their colds had no effect on their metabolic responses.

Another question was: Does exercising when you have a cold affect your symptoms and recovery time?

Once again, Dr, Kaminsky and his colleagues infected volunteers with a rhinovirus. This time, the subjects were 34 young men and women who were randomly assigned to a group that would exercise with their colds and 16 others who were assigned to rest.

The group that exercised ran on treadmills for 40 minutes every other day at moderate levels of 70 percent of their maximum heart rates.

Every 12 hours, all the subjects in the study completed questionnaires about their symptoms and physical activity. The researchers collected the subjects’ used facial tissues, weighing them to assess their cold symptoms.

The investigators found no difference in symptoms between the group that exercised and the one that rested. And there was no difference in the time it took to recover from the colds. But when the exercisers assessed their symptoms, Dr. Kaminsky said, “people said they felt O.K. and, in some cases, they actually felt better.”

Now, Dr. Kaminsky said, he and others at Ball State encourage people to exercise when they have colds, at least if they have the type producing symptoms like runny noses and sneezing. He is more cautious about other types of colds that produce fevers or symptoms below the neck such as chest congestion. Exercising with a head cold is not an issue for athletes, Dr. Kaminsky said, because most of them want to train no matter what. “If anything they tend to push too much,” he said.

Dr. Kaminsky also runs a fitness program at the university, dealing with regular exercisers. When he tells them it is all right to exercise when they have a cold, many are “a little suspicious,” he said. Often, they want to back off a little, lowering the intensity of their efforts.

“We tell them that’s O.K. if it’s for a short period of time,” Dr. Kaminsky said. “But what you have to be cautious of, where I see it as more of an issue, is with people who are trying to build that exercise habit. They’ve got all these barriers anyway.”

AND too often taking time off because of a cold is the start of falling away from the program entirely.

Dr. Kaminsky, who runs and works out on elliptical cross trainers and does resistance training, takes the studies’ findings to heart. Now when he has a cold, he continues to work out.

“It did give me the personal assurance that it was a good thing to do,” he said.

Low-carb? Low-fat? Study finds calories count more

Low-fat, low-carb or high-protein? The kind of diet doesn’t matter, scientists say. All that really counts is cutting calories and sticking with it, according to a federal study that followed people for two years. However, participants had trouble staying with a single approach that long and the weight loss was modest for most.

As the world grapples with rising obesity, millions have turned to popular diets like Atkins, Zone and Ornish that tout the benefits of one nutrient over another.

Some previous studies have found that low carbohydrate diets like Atkins work better than a traditional low-fat diet. But the new research found that the key to losing weight boiled down to a basic rule — calories in, calories out.

“The hidden secret is it doesn’t matter if you focus on low-fat or low-carb,” said Dr. Elizabeth Nabel, director of the National Heart, Lung and Blood Institute, which funded the research.

Limiting the calories you consume and burning off more calories with exercise is key, she said.

The study, which appears in Thursday’s New England Journal of Medicine, was led by Harvard School of Public Health and Pennington Biomedical Research Center in Louisiana.

Researchers randomly assigned 811 overweight adults to one of four diets, each of which contained different levels of fat, protein and carbohydrates.

Though the diets were twists on commercial plans, the study did not directly compare popular diets. The four diets contained healthy fats, were high in whole grains, fruits and vegetables and were low in cholesterol.

Nearly two-thirds of the participants were women. Each dieter was encouraged to slash 750 calories a day from their diet, exercise 90 minutes a week, keep an online food diary and meet regularly with diet counselors to chart their progress.

There was no winner among the different diets; reduction in weight and waist size were similar in all groups.

People lost 13 pounds on average at six months, but all groups saw their weight creep back up after a year. At two years, the average weight loss was about 9 pounds while waistlines shrank an average of 2 inches. Only 15 percent of dieters achieved a weight-loss reduction of 10 percent or more of their starting weight.

Dieters who got regular counseling saw better results. Those who attended most meetings shed more pounds than those who did not — 22 pounds compared with the average 9 pound loss.

Lead researcher Dr. Frank Sacks of Harvard said a restricted calorie diet gives people greater food choices, making the diet less monotonous.

“They just need to focus on how much they’re eating,” he said.

Sacks said the trick is finding a healthy diet that is tasty and that people will stick with over time.

Before Debbie Mayer, 52, enrolled in the study, she was a “stress eater” who would snack all day and had no sense of portion control. Mayer used to run marathons in her 30s, but health problems prevented her from doing much exercise in recent years.

Mayer tinkered with different diets — Weight Watchers, Atkins, South Beach — with little success.

“I’ve been battling my weight all my life. I just needed more structure,” said Mayer, of Brockton, Mass., who works with the elderly.

Mayer was assigned to a low-fat, high-protein diet with 1,400 calories a day. She started measuring her food and went back to the gym. The 5-foot Mayer started at 179 pounds and dropped 50 pounds to 129 pounds by the end of the study. She now weighs 132 and wants to shed a few more pounds.

Another study volunteer, Rudy Termini, a 69-year-old retiree from Cambridge, Mass., credits keeping a food diary for his 22-pound success. Termini said before participating in the study he would wolf down 2,500 calories a day. But sticking to an 1,800-calorie high-fat, average protein diet meant no longer eating an entire T-bone steak for dinner. Instead, he now eats only a 4-ounce steak.

“I was just oblivious to how many calories I was having,” said the 5-foot-11-inch Termini, who dropped from 195 to 173 pounds. “I really used to just eat everything and anything in sight.”

Dr. David Katz of the Yale Prevention Research Center and author of several weight control books, said the results should not be viewed as an endorsement of fad diets that promote one nutrient over another.

The study compared high quality, heart healthy diets and “not the gimmicky popular versions,” said Katz, who had no role in the study. Some popular low-carb diets tend to be low in fiber and have a relatively high intake of saturated fat, he said.

Other experts were bothered that the dieters couldn’t keep the weight off even with close monitoring and a support system.

“Even these highly motivated, intelligent participants who were coached by expert professionals could not achieve the weight losses needed to reverse the obesity epidemic,” Martijn Katan of Amsterdam’s Free University wrote in an accompanying editorial.
